Wellness
- Wellness committee sponsored by C-Suite Executive
- Biometric event (waived if wellVibe is utilized)
- No tobacco policy at work; 90% coinsurance or copays after 100% coinsurance
- Written business plan
- Contribution strategy that incents biometric event participation and health assessment completion
Optimal Plan Components
- Meet definition of UnitedHealthcare Optimal Prescription (Rx)
- Emergency Room (ER) copay ≥$150
- ER and Urgent Care spread in copay ≥$100
- Consumer-Driven Health Plan (CDHP) design with co-insurance ≥90%
- CDHP account funding ≤75%
Outcome-Based Plan Design
- Implement benefits strategy that provides significant incentive around meeting outcome goals for
blood pressure, cholesterol, non-tobacco use and BMI.
- SimplyEngaged® Plus, or other like program
- Must achieve plan participation of 75% of all eligible employees
Diabetes Prevention & Control Alliance (DPCA)
- Promotion of the DPCA Program
- Sponsor glucose or HA1C screening
- Allow onsite counseling when applicable
